Work collaboratively with multidisciplinary teams to optimize drug therapy.
Provide direct patient care, including treatment assessment and medication management activities.
Clinically evaluate all patients in specified area on a daily basis with relevant documentation placed in the medical record regarding pharmacy recommendations, plans, interventions, and medication reconciliation.
Review the patient’s drug regimen for appropriate indication, dosing, allergy verification, interactions potential, lab monitoring, duplication of therapy, drug interactions and medication documentation.
Provide cost-effective management of drug utilization within the clinic.
Monitor process and outcome of patient care relating to drug therapy.
Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects.
Qualifications
Internal Medicine Pharmacist Qualifications:
Degree/Education: Bachelor's degree in pharmacy. Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education
Certifications: Basic Life Support
Experience: A minimum of one year pharmacy experience within the last 3 years. Completion of an ASHP accredited PGY-1 Pharmacy Practice Residency or equivalent experience is required. Board certification is preferred. Chief, Department of Pharmacy has option to waive requirement
