A wonderful opportunity to impact patients’ lives in a team-based primary care practice that integrates clinical care and education. Part of the General Medicine Division at URMC, Strong Internal Medicine’s (SIM) purpose is to help its diverse patients live full and healthy lives – and the practice is equipped to do so. An NCQA-recognized Patient Centered Medical Home, SIM’s highly team-based care model includes advanced practice providers, attending physicians, internal medicine residents, registered nurses, social workers, behavioral health providers, clinical pharmacist, registered dietitian, care managers, and administrative support. The practice emphasizes a team-based approach, where interdisciplinary collaboration is key, and the roles of team members like behavioral health providers and clinical pharmacists are integral to the collaborative nature of the practice.
SIM is looking for an APP with a passion for improving health outcomes for diverse patient populations who oftentimes have challenging medical and social circumstances. A critical and valued part of the care team, the APP ensures continuity of care for patients. Direct care includes the evaluation and management of acute and chronic conditions in clinic. An autonomous provider, the APP will bill for clinic services. Indirect care includes coordination with the team, follow-up on lab results, patient calls/MyChart messages, and managing patients on anticoagulation medication. We’re looking for a compassionate individual, preferably with 2 years of experience as a Nurse Practitioner/Physician Assistant in an ambulatory care environment. Our APP must also have excellent communication skills, empathy, and ability to thrive in a fast-paced, multidisciplinary team-based environment. Current certification through NYS as an NP/PA required. The APP will also participate in the education of learners (students and new employees), using clinical learning experiences, knowledge sharing, and skill training. Ability to meet productivity targets.

Qualifications:
Nurse Practitioner
Require a Master's degree as a Nurse Practitioner, an active New York State Registered Nurse license in good standing, an active New York State Nurse Practitioner licensure in good standing and current national certification as an Nurse Practitioner.
Physician Assistant
Required completion of an ARC-PA approved Physician Assistant program, an active New York State Physician Assistant licensure in good standing and current national certification as a Physician Assistant by NCCPA (National Commission on Certification of Physician Assistants)
NPs and PAs will demonstrate clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn. The APP will participate in the education of learners (students and new employees), using clinical learning experiences, knowledge sharing and skill training.
